The CarePathOnLine Support Center is part of the CarePath Health Information Service provided by DIANON Systems, a subsidiary of Laboratory Corporation of America. DIANON Systems is responsible for the information provided on this Web site. We have created this privacy policy because we respect your right to keep personal information private. This statement explains our practices for collecting and using information from this Web site.

Our Guiding Principles
We recognize that a person’s health status is among the most personal of all issues, and we believe that information about your health should remain private unless you choose to share it. In keeping with this belief, our Web site gives you access to all of its information and resources without requiring you to provide any personal information about your health. The only information that CarePathOnLine may obtain about you is that which you choose to provide.

In some cases, we may need personal information (such as your e-mail address) in order to inform you about new content or services on CarePathOnLine or to provide customized information upon your request. In these cases, we will ask explicitly for the information we need to fulfill your request, and you will always have the option not to provide it. We will never willfully disclose personally identifiable information that you have provided (such as your name or e-mail address) to any third party without first receiving your permission.

Only statistical information about our visitors as a group (for example, the average number of Web pages viewed, the most popular pages on our site, etc.) may be shared with a third party. Information you may have provided to us that identifies you personally will not be shared at any time without your prior permission.

We want to make our Web site as helpful as possible to you, your family, and your health care team as you make important decisions about your health care. You may choose to help us by sharing your comments about our Web site and your suggestions for enhancements. The Visitor Survey provides a quick, easy way for you to do this, or you may choose to communicate with us via e-mail by sending Questions or Comments. Neither method requires you to provide personal information. If you choose to provide your name and/or e-mail address, we will safeguard this personal information as explained in this policy. CarePathOnLine stores feedback that users send us. It is used to administer and refine the CarePathOnLine service and, at times, may be shared with CarePathOnLine partners, either in aggregate form, which cannot be traced to an individual user, or with personal information (such as visitors’ names and e-mail addresses) removed.

E-mail Notification of New Information or Services
We add new information and services to this Web site on a regular basis. You may choose to provide your e-mail address so that we may inform you of these enhancements. If you do, we will safeguard your e-mail address as explained in this policy and will not use it for any other purpose.

Site Visitor Information
We are always looking for ways to serve you better and improve the offerings on our Web site. CarePathOnLine requires its vendors to adhere to our confidentiality standards and will not permit a vendor to use our visitor information for any purpose.

In certain limited circumstances, CarePathOnLine may be legally compelled to release your personal information in response to a court order, subpoena, search warrant, law, or regulation. Our Relationship to Third Parties
CarePathOnLine features links to other Web sites that we believe you might find useful. We have reviewed these sites and have done our best to list only reputable sites with sound business practices. However, it is important for you to understand that we do not influence or control these external Web sites, and we are not responsible for their privacy practices. We will notify you when you are leaving our site. Your activity on external Web sites is covered by the privacy policy of those sites. We encourage you to read the privacy policy of each site you visit that collects information or asks you to disclose personal and/or health-related personal information.

The statements in this privacy policy apply solely to information collected on CarePathOnLine

Our Children’s Policy
CarePathOnLine recognizes that certain sections of our site may feature information of particular interest to children under the age of 13. In fact, some sections of our site, such as the Children’s section of the Family Room, have been created specifically to help young users better understand complex health issues that they or their family members may face. This information is available to all users on an anonymous basis, just as it is in other areas of our site. To protect user privacy, no personal information is collected.

CarePathOnLine does not request or collect personal or health-related personal information about visitors to the Children’s section of the Family Room, to include our under-13 users. However, we may use information about users’ visits to the Children’s Section to create aggregate data that may be shared with third parties. Aggregate data cannot be traced to an individual user.

Our Security Measures
CarePathOnLine wants any personal information you choose to provide to us to remain as secure as reasonably possible. Personal information obtained from visitors to this site is stored on a server that is physically separate from the one that contains the rest of the CarePathOnLine content. The server is protected by firewalls that meet or exceed industry standards to prevent intruders from gaining access to the data they contain.

Although we will make every reasonable effort to protect personal information from loss, misuse or alteration by third parties, you should be aware that there is always some risk involved in transmitting information or that thieves will find a way to thwart our security systems.

CarePathOnLine reserves the right to change the terms of this privacy policy at any time by posting revisions to this Web site. If we ever decide to use personal information or health-related personal information in a manner different from that stated at the time it was collected, we would notify you by e-mail.
